Purchase requests were approved over email. Requests stalled when an approver was travelling, nobody could say where a given request had stopped, and there was no reliable record afterwards.

Multi-step routing with value-based thresholds, automatic delegation when an approver is unavailable, and time-based escalation. Every state change is recorded against the request.
